New Patient Registration
When registering, please bring your NHS Medical card to the Practice and those of any other persons registering with you. We also require proof of your address, a recent utility bill or rent/mortgage agreement, and photographic ID. You will be asked to complete a registration form for each person registering with the Practice.
If you are newly resident in the UK or you are an overseas visitor then additional documentation is required – contact our reception team for more information.
In addition, all patients will be asked to complete a new patient questionnaire, allowing us to provide medical care in the interim period, while your medical records are transferred from your old practice to this one. If you move out of the practice area, you will need to re-register with a Practice in your area.
Patients are registered with practice, not an individual GP. For administrative reasons your medical card will be issued in the name of one of the doctors; however you can at any time express a preference for a particular doctor, for either all of your medical needs or on a case-by-case basis. This preference will be recorded in your medical notes and we will do our best to respect your choice. However, not all the doctors in the practice provide all services.
The boundary area for the Practice is as follows:
Along London Road to the top of Easter Road, down Easter Road (right side only), along East Hermitage Place and Claremont Park then onto Seafield Road along Seafield Road East and onto Sir Harry Lauder Road, turning right onto Milton Road West and Duddingston Road, through the village and along Queens Drive, past Arthur Seat then back onto London Road.
